A Doctor of Physical Therapy specializing in home health care since 2010. Loves sports, photography, traveling and eating! Forever fan of MJ, Kobe and KD! Amateur photographer, wishing to be a pro, too (maybe!) Still loves playing video games. Never formal and serious until the situation absolutely calls for it. Prefers sneakers over leather shoes, shorts over pants, comfy pants over jeans. A proud parent of an amazing and beautiful boy, hoping to be a parent of more children, too! Lastly, no matter what life brings, Badz will always be a kid at heart.

Angelica GriffithCLIENT SUPPORT SERVICES
A true chaos coordinator in real life (daughter, sister, friend, wife, mother of real baby and a fur baby), Angelica Griffith finds beauty and art in chaos. She’s CODE4Five’s mighty Admin Officer. She tries her best to wear as many hats as she can liaising with CODE4Five’s internal (CODE4Five team!) and external business partners (our dear clients!), to make the operations seamless, professional and orderly as much as possible. She’s a Communication Arts graduate, was an adventure traveler but now enjoys the role of finding sense and rationality in chaotic situations. By the way, her middle name is Dependability. Say hi to CODE4Five’s go-to-lady!

Cheryl MeyerRN, COS-C, HCS-D
Having 35 valuable years in the field of Nursing, 25 noteworthy years in Home Health, including management/directorial/trainer positions in both fields, blessed with a wonderful husband, a certified-electrician son and her daughter in grad school pursuing to be an Occupational Therapist, one would think a fulfilled woman like Cheryl will just retire in the Bahamas. But she’s not your run-of-the-mill kind of person. She found her passion continuing her legacy in her career by doing remote Quality Audit job from home for Home Health agencies. Her vast knowledge and her passion for auditing and coding had driven her to persistently contribute in the improvement of clinical documentation, compliance to CMS rules and guidelines and coding. Cheryl joined CODE4Five in its pilot year and had been a “gem” since then. She holds under her belt certifications as an OASIS Specialist (COS-C) and ICD-10 (HCS-D). Cheryl balances “life” by going camping, gardening, crafts and game nights with family, friends and their “Empty Nest dog” Cole.

Karen SmithRN, COS-C, BCHH-C
Karen Smith joined CODE4Five in 2018, but her involvement with home health started many years ago. She has been learning and teaching about OASIS since its inception. Karen has played many parts in home health – Case Manager, staff educator, diagnosis/OASIS coder, clinical supervisor, but the best is being a QA for Code4Five! In this role she can educate home health clients as well as play her favorite game of “Find the Best Codes”. To keep sane in the wild world of home health, she spends time in the beautiful outdoors of her Idaho home, or volunteering at the local animal shelter.
